If your schedule changes and you cannot make your appointment for any reason, please contact us, via phone at (408) 214-5559, so we may reschedule you, and so that we may accommodate those patients who are waiting for an appointment. Changes to your appointment must be made at least 24 hours before your scheduled appointment. If you are unable to cancel or reschedule your appointment with at least 24 hours’ notice, please be advised that we may assess a $25 “no-show” charge that will be directly billed to you.
The privacy of your medical information is important to us and we are committed to protecting it. Medical records are for the benefit of the patient and serve as a note to help you and your practitioner document the services and care you received at our clinic and to comply with legal requirements.
We will not use or disclose your medical information for any purpose other than for treatment, payment, healthcare operations or when required by law unless authorized with your written permission. Any permission given may be revoked at any time by writing to us. Your contact information given in your health records may be used to send birthday cards, newsletters and appointment reminders via phone call, text or mail.
Upon written request, you have the right to access, review or receive copies of your health care records. We charge a copy fee of $15, please allow 10 working days to process the request.
Licensed acupuncturists (LAc) are considered Primary Care Providers (PCP) in the state of California, meaning that licensed acupuncturists have the ability to practice independently, serve as a primary care provider and as necessary collaborators with other health care providers. Though we are happy to serve as your primary care provider, it is important to acknowledge that patients benefit the most when receiving care from a team of healthcare providers. Please continue to maintain a close relationship with your medical doctor (MD or DO) in conjunction with utilizing the services of a TCM doctor. In the event that a patient wishes for their medical doctor or other health care provider to communicate or collaborate with their acupuncturists, be sure to ask for a release of medical records so that your records may to transferred with your privacy and security in mind.
Cell phones are communication miracles but can easily become an obstacle to mind-body wellness. While receiving acupuncture or any other TCM services, it's important to tap into a introspective and self-centric mindset.
